H&W: Premium Assistance Under Medicaid and the Children’s Health Insurance Program

If you or your children are eligible for Medicaid or the Children’s Health Insurance Program (CHIP) and you’re eligible for health coverage through your employer, your state may have a premium assistance program that can help pay for premium expenses, using funds from their Medicaid or CHIP programs. If you or your children aren’t eligible for Medicaid or CHIP, you won’t be eligible for these premium assistance programs but you may be able to buy individual insurance coverage through the Health Insurance Marketplace. H&W: Are All Eligible Participants Enrolled in the Fund’s Life Insurance Benefit?

Professional Musicians & Employer’s Health & Welfare Fund:

The Professional Musicians Local 47 and Employers’ Health & Welfare Fund (“Fund”) provides a $20,000* Life Insurance benefit to all Participants who have qualified for benefits. Completion of an enrollment form is not required and enrollment is automatic once the Fund confirms, after the close of the annual qualifying window (generally Oct. 3 of every year), that you have qualified for benefits based on the employer contributions made on your behalf in the prior qualifying year (generally Oct. 2 through Oct. 3). Contributions in the qualifying year, determine eligibility for the following January through December (i.e. employer contributions made during the period of Oct. 3, 2016 through Oct. 2, 2017, have determined eligibility for the period Jan. 1, 2018 through Dec. 31, 2018). H&W: Eligibility & Enrollment Information

Professional Musicians, Local 47 and Employers’ Health & Welfare Fund

The Fund mailed Eligibility and Enrollment information to all eligible participants in mid-November. The enrollment materials advise each participant of the level at which they qualified (A, B or C).

The qualifying levels are as follows:

Level             Employer Contribution Amount
A                    $1,500 plus
B                    $1,200 to $1,499
C                    $800 to $1,199

In order to be covered by one of the Fund’s benefit plans, for the coverage year beginning January 1, 2018, you must (A) have accumulated a minimum of $800 in employer contributions, for covered work performed between October 3, 2016 through October 2, 2017, and (B) remit, to the Fund’s Administrative Office, a completed enrollment form along with the proper co-premium, no later than December 20, 2017. The Professional Musicians Local 47 and Employers’ Health & Welfare Fund (“Fund”) will soon be determining eligibility for benefit coverage effective January 1, 2018. Important Contribution and enrollment information will be mailed out by the Fund; if you don’t complete and return this information on a timely basis it could negatively affect your ability to enroll or qualify for coverage with the Fund. The Professional Musicians Local 47 and Employers Health and Welfare Fund (“the Fund”) purchases health insurance benefits through Health Net of California and provides you with this notice as required by the Women’s Health and Cancer Rights Act of 1988 (WHCRA).

In compliance with WHCRA, Health Net provides benefits for mastectomy-related services, including all stages of reconstruction and surgery to achieve symmetry between the breasts, prostheses, and for complications resulting from a mastectomy, including lymph edemas. Continue reading
