Open Enrollment runs from 12/05/2016 to 12/31/2016 at 12:00pm PT. The fastest

If you are covered by the Flex Plan:
The group numbers for the Plan 60 and Plan 80 are changing effective January 1,
2017. Please go through the Open Enrollment Process and you will be sent a new
ID card with the new group numbers.
If you are not covered by the Flex Plan:
All participants must go through the Open Enrollment Process whether you have
verification form each year. The verification form is generated by going
through Open Enrollment. In order to be eligible for claims reimbursement,
your 2017 insurance verification form must be on file.
If you receive Self-Payment Billings from the Flex Plan, please consider
setting up pre-tax withholdings from payroll to cover the difference between
your employer contributions and the cost of your premiums. Pre-tax elections
can be set up on-line after completing Open Enrollment or by speaking to a
Member Services Representative after you enroll. Not all employers allow
pre-tax withholdings; to see if your employer offers pre-tax withholdings, call
Member Services. Pre-tax elections CANNOT be accepted after 12/31/2016.
