INSTRUCTIONS:

1. To join the Local 47 Referral Service, members must register with the Referral Service in addition to their general Local 47 membership. Registration with the Referral Service is always free and occurs annually.

2. To register or re-register with The Referral Service, Local 47 members must sign and date The 2009 Referral Service Rules & Regulations document before December 31, 2008 to insure their active status for the 2009 calendar year.

3. Once this document is signed, please return it with an intact handwritten signature in one of the following ways:

* as a scanned attachment in .jpg or .pdf format to referrals@promusic47.org

* by fax to 323-469-5866, attn: Referral Service
