INSTRUCTIONS:

To participate in the Local 47 Referral Service -- a free program for Local 47 members -- you must annually register with the Referral Service and keep their Local 47 membership current. Registration with the Referral Service is always free and occurs Dec. 31 of each year.

To register or re-register with the Referral Service, Local 47 members must sign and date (1) the Referral Service Rules & Regulations document and (2) the Referral Service Website Agreement before Dec. 31 to ensure active status for the next calendar year.

Once these two documents are signed, please return them with an intact handwritten signature in one of the following ways:
