Changes Ahead For Life/Dental/Vision Coverages

2020 PARTICIPANT ELIGIBILITY LEVELS

Commencing with the January 1, 2020, coverage year, the minimum initial eligibility threshold to qualify for enrollment in medical plans sponsored by the Professional Musicians, Local 47 and Employers’ Health & Welfare Fund (the “Fund”) will be reduced to $700 in Employer contributions. Thus, effective with the current Qualifying Period — October 2, 2018 through October 3, 2019 — the amount of Employer contributions necessary for eligibility at the Fund’s “C Level” of coverage/benefits shall be $700 (including any available permissible carry-forwards from the 2017/2018 Qualifying Year). Reconstructive Surgery Fully Covered After Mastectomy

WOMEN’S HEALTH AND CANCER RIGHTS ACT OF 1998

The Professional Musicians Local 47 and Employers Health & Welfare Fund (“the Fund”) provides you with this notice as required by the Women’s Health and Cancer Rights Act of 1988 (WHCRA).

In compliance with WHCRA, the insurers contracted with by the Fund (Kaiser and Blue Shield) provide benefits for mastectomy-related services, including all stages of reconstruction and surgery to achieve symmetry between the breasts, prostheses, and for complications resulting from a mastectomy, including lymph edemas. The Professional Musicians Local 47 and Employers’ Health & Welfare Fund (“Fund”) will soon be determining eligibility for benefit coverage effective January 1, 2019. Important Contribution and enrollment information will be mailed out by the Fund; if you don’t complete and return this information on a timely basis it could negatively affect your ability to enroll or qualify for coverage with the Fund. H&W: Premium Assistance Under Medicaid and the Children’s Health Insurance Program

Professional Musicians & Employer’s Health & Welfare Fund:

If you or your children are eligible for Medicaid or the Children’s Health Insurance Program (CHIP) and you’re eligible for health coverage through your employer, your state may have a premium assistance program that can help pay for premium expenses, using funds from their Medicaid or CHIP programs. If you or your children aren’t eligible for Medicaid or CHIP, you won’t be eligible for these premium assistance programs but you may be able to buy individual insurance coverage through the Health Insurance Marketplace. H&W: Are All Eligible Participants Enrolled in the Fund’s Life Insurance Benefit?

Professional Musicians & Employer’s Health & Welfare Fund:

The Professional Musicians Local 47 and Employers’ Health & Welfare Fund (“Fund”) provides a $20,000* Life Insurance benefit to all Participants who have qualified for benefits. Completion of an enrollment form is not required and enrollment is automatic once the Fund confirms, after the close of the annual qualifying window (generally Oct. 3 of every year), that you have qualified for benefits based on the employer contributions made on your behalf in the prior qualifying year (generally Oct. 2 through Oct. 3). Contributions in the qualifying year, determine eligibility for the following January through December (i.e. employer contributions made during the period of Oct. 3, 2016 through Oct. 2, 2017, have determined eligibility for the period Jan. 1, 2018 through Dec. 31, 2018).
