The Professional Musicians Local 47 and Employers Health and Welfare Fund (“the Fund”) purchases health insurance benefits through Health Net of California and provides you with this notice as required by the Women’s Health and Cancer Rights Act of 1988 (WHCRA).

In compliance with WHCRA, Health Net provides benefits for mastectomy-related services, including all stages of reconstruction and surgery to achieve symmetry between the breasts, prostheses, and for complications resulting from a mastectomy, including lymph edemas. Continue reading

November 20th – December 20th

Enrollment information has been mailed to all eligible participants. November 20th through December 20th is the Fund’s Open-Enrollment period. This is the time of year to select one of the benefit options if you are newly eligible, and decide to enroll, or to make changes to your benefits if you are already enrolled and remain eligible for the same level of coverage. Eligibility & Enrollment Information

 Eligibility and Enrollment information will be mailed to eligible participants in approximately mid-November. Enrollment materials will be sent to the participant’s last address on file with the Fund’s Administrative Office, so please ensure that the Fund has your most up-to-date address information.

The enrollment materials will advise the eligible participant at which Level of coverage they have qualified (A, B or C), based on employer contributions the Fund received on the participant’s behalf for employment between Oct. 3, 2015, and Oct. 2, 2016. Benefit summaries and co-premium costs for the applicable Level will also be included in the enrollment material. Continue reading

HAVE YOU HAD A CHANGE OF ADDRESS?

The Professional Musicians Local 47 and Employers’ Health & Welfare Fund (“Fund”) will soon be determining eligibility for benefit coverage effective January 1, 2017 through December 31, 2017. Very important contribution and enrollment information will be mailed out by the Fund to each participant of record and musicians who have qualified for coverage based on employer contributions for work performed during the Fund’s 2015/16 Qualifying Period (October 3, 2015 through October 2, 2016).
