WOMEN’S HEALTH AND CANCER RIGHTS ACT OF 1998

The Professional Musicians Local 47 and Employers Health and Welfare Fund (“the Fund”) purchases health insurance benefits through Health Net of California and provides you with this notice as required by the Women’s Health and Cancer Rights Act of 1988 (WHCRA).

In compliance with WHCRA, Health Net provides benefits for mastectomy-related services, including all stages of reconstruction and surgery to achieve symmetry between the breasts, prostheses, and for complications resulting from a mastectomy, including lymph edemas.
Although reconstructive surgery after a mastectomy is generally a covered procedure, some procedures are, of course, classified as cosmetic and, therefore, not covered. Continue reading

H&W: Premium Assistance Under Medicaid and the Children’s Health Insurance Program (CHIP)

H&WIf you or your children are eligible for Medicaid or CHIP and even if you are eligible for health coverage from your employer – in our case, the Professional Musicians, Local 47, and Employers’ Health & Welfare Fund (the “Fund”) – your state may have a premium assistance program that can help pay for your children’s coverage, using funds from the that State’s Medicaid or CHIP programs. If you or your children aren’t eligible for Medicaid or CHIP, you won’t be eligible for these premium assistance programs but, nonetheless, you may be able to buy them (and yourself, if you have not qualified for coverage through the Fund) individual insurance coverage through the Health Insurance Marketplace. Fund must receive your completed application prior to Dec. 8

The Professional Musicians Local 47 and Employers’ Health & Welfare Fund (“Fund”) has determined eligibility for benefit coverage effective January 1, 2016. Important enrollment information has been mailed out by the Fund to all eligible members. If you don’t complete and return this information on a timely basis it could negatively affect your ability to enroll or qualify for coverage with the Fund. A delay in the submission of your enrollment form and co-premium may affect your coverage and benefits. Continue reading
